Population Health Management
The challenge in today’s delivery of health care is to identify the patient that will become ill and assist that patient in the care process before the illness escalates out of control.
Today’s healthcare providers confront increasing challenges in the delivery of high quality health care services. The population is aging, incidences of chronic diseases are increasing, and providers must ensure they are delivering the best possible care.
To help physicians meet the challenge, MED3OOO’s Population Health Management program seeks to identify patient health needs and improve the health status of the patient populations managed by physician practices. MED3OOO has laid the foundation for Population Health Management by acquiring the right tools and methodologies to identify patients who are chronically ill, as well as those who are at future risk for generating high cost health care expenditures.
The goal of MED3OOO’s Population Health Management program is to manage the health of a defined population optimally by providing the right intervention to the right people at the right time. MED3OOO’s Population Health Management program uses customized sets of interactions to address each person’s unique set of healthcare risks and conditions. The program tailors interventions to ensure the patient’s acceptance and effectiveness.
One of the major challenges in health care is to identify patients who will become ill in the future and assist those patients in the care process before the illness escalates out of control. MED3OOO utilizes a unique software system to identify patients at risk for future heath care expenditures that are greater than the norm. We then provide medical management systems to our IPA and TPA clients with customized clinical guidelines for providers to use during medical management activities of high risk patients.
MED3OOO focuses resources where they can have the greatest impact by identifying the right people in need of those resources at the time that they need them. Our focus is to anticipate and manage future health care needs because we know that today’s heavy utilizers are not necessarily the high cost utilizers of tomorrow.
