Case and Disease Management

MED3OOO case management encompasses a continuum of inpatient and outpatient services on a case-by-case basis to achieve realistic treatment goals.  Our experienced case managers monitor inpatient admissions to determine appropriate discharge dates and arrange for continuing care needs after discharge.  Outpatient resources are arranged and coordinated, such as home health services and durable medical equipment, for patients whose care needs can be met on an outpatient basis or who may have long-term care needs due to chronic illness or injury.  MED3OOO case managers collaborate with all members of the health care team, including discharge planners, UM staff at affiliated hospitals, case managers at affiliated health plans, treating physicians, and ancillary providers to ensure your patient’s care is coordinated and facilitated in the most efficient, cost-effective manner possible.
 
MED3OOO disease management staff assist your patients in enrolling in disease management programs offered by their health plans.  It is the goal of our disease management interventions to encourage IPA patients to adopt healthier lifestyles by becoming more aware of their condition and taking greater responsibility for participating in the management of their health.  MED3OOO disease management integrates primary and specialty services, educational and self-management services, and ongoing monitoring of clinical progress.  Our case managers assess your patient’s long-range care needs, make arrangements for support services and supplies, and monitor your patient’s progress until released from treatment.  It is our goal to assist providers in improving the health of their patients through proactive, hands-on disease management.
